Efficacy of intravenous ketamine as premedicant for prevention of intraoperative hypotension after spinal anesthesia in parturients posted for elective cesarean section

Background: Hypotension following spinal anesthesia (SA) in lower segment cesarean section can be deleterious to the parturient and fetus. Ketamine acts by stimulating cardiovascular system leading high blood pressure increase heart rate (HR) cardiac output. Aims Objectives: This study aimed assess efficacy of 15 mg prophylactic intravenous ketamine elective for prevention hypotension after SA. Materials Methods: double-blinded randomized controlled was conducted on 98 participating parturients who were American Society Anesthesiologists term pregnant women. The randomly divided into two groups (n=49 each). All received group - Group K IV bolus 3 mL saline control C same volume normal bolus. HR systolic (SBP) recorded at baseline 5, 10, 15, 20 min injection then every till end operation. Incidences recorded. total number dose ephedrine used, APGAR score, Visual Analog Scale scores Ramsay Sedation Score also baseline, 30, 45 injection, Results: showed a statistically significant incidence higher use 6 boluses when compared group. Forty-two developed 23 with P<0.001. significantly among observed first intraoperatively P=0.001. There no difference score between groups. Conclusion: hemodynamic stability regard SBP, diastolic pressure, HR. Hence, low-dose ketamine, used as premedicant, is an effective agent that preventing post-spinal intraoperative undergoing delivery.
